[The investigation report on the "12·24" large coal and gas outburst accident at the Zhenxiong Daying Coal Mine in Yunnan was released, and 40 responsible persons were held accountable.] On October 8, the Yunnan Bureau of the State Administration of Mine Safety recently released the "Investigation Report on the "12·24" large coal and gas outburst accident at Zhaotong Zhenxiong Daying Coal Mine Co., Ltd." After investigation, it was determined that the accident was a production safety liability accident caused by the failure of the company to fulfill its main responsibilities and the ineffective local coal mine safety supervision. At about 20:01 on December 24, 2025, a coal and gas outburst accident occurred in Daying Coal Mine of Daying Coal Mine Co., Ltd. in Zhenxiong County, Zhaotong City (hereinafter referred to as Daying Coal Mine), killing 7 people and causing direct economic losses of 15.962 million yuan. Through on-site inspection, investigation and evidence collection, data review, and comprehensive analysis, we found out the occurrence, cause, casualties, and direct economic losses of the accident, identified the nature and responsibility of the accident, and put forward handling suggestions for relevant responsible personnel and responsible units in accordance with laws and regulations. We also proposed accident prevention and rectification measures based on the cause of the accident and outstanding problems exposed. The accident site is located directly in front of the excavation working face of the 1503 transport lane. After the accident, the Zhaotong Municipal People's Government promptly launched an emergency plan, established an emergency rescue headquarters, and carried out comprehensive on-site rescue work. After all-out rescue, at 15:32 on December 27, all seven victims were found and the rescue ended. After technical appraisal, the amount of coal and rock outburst in this coal and gas outburst accident was 591 tons, and the total amount of outburst gas was 55,300 cubic meters. The investigation found that after the accident, underground workers actively carried out self-rescue and mutual rescue, and Daying Coal Mine conducted early emergency response, but did not summon the full-time (part-time) mine rescue team at the first time. Some of the rescue equipment of the part-time mine rescue team failed and the assembly took 57 minutes. There were problems such as untimely emergency response and insufficient emergency preparation. The investigation determined that the "12·24" large coal and gas outburst accident in Daying Coal Mine was a production safety liability accident caused by the failure to implement the main responsibilities of the enterprise, inadequate safety supervision of local coal mines, failure to follow outburst coal seam management after the gas dynamic phenomenon occurred in the coal mine, and excavation operations without eliminating the risk of outbursts. The accident investigation team made recommendations on accountability for the 40 responsible personnel. Among them, 5 persons, including the legal representative and mine director of Daying Coal Mine, were handed over to the judicial authorities for handling according to law; 11 persons, including the deputy director of safety and deputy director of production, were given penalties such as suspension or revocation of safety production knowledge and management ability assessment certificates, and administrative fines; 2 legal representatives and general managers of Donghuang Coal Mine were given administrative fines; 2 people from the coal mine safety production technical service agency were given administrative fines; 20 persons responsible for the accident, including the Zhaotong Energy Bureau and Zhenxiong County, were given party discipline and government sanctions and organizational handling. Daying Coal Mine was given an administrative fine, its coal mine safety production license was temporarily withheld, and it was ordered to suspend production for rectification and carry out intelligent transformation. Administrative fines were imposed on Donghuang Coal Mining Company. Administrative fines will be imposed on coal mine safety production technical service agencies and ordered to suspend operations for rectification. Zhenxiong County Energy Bureau, Zhaotong City Energy Bureau, Zhenxiong County Party Committee, and County People's Government were ordered to make written inspections. (CCTV News)
